Rider Applications

We are accepting applications for our wait list, as we are currently at full capacity and can only accept new riders as vacancies occur. If you or someone you know may be appropriate for our program, you can download application forms using the links below. (Requires Adobe Acrobat. If Acrobat is not on your computer, you can download a free copy here.)

Please note that the minimum age for TROT riders is four; there is no upper age limit. Please be sure to also download the Physician Statement form. This must be completed by the lead health care provider for the applicant. We cannot process an application packet until all forms are received. Reception of a completed packet does not constitute enrollment. We will contact you with further information.
